1. Collection of Personal Information
This site does not contain its own form for directly submitting personal information. When you contact us by email, we receive the sender’s email address and the information included in the message. When you use Google Forms, we receive the information you enter and submit. Whether a Google Form collects an email address depends on its settings; check the notice displayed on the form before submitting. The web server and external services may process technical information such as IP address, browser information, and access date and time.
2. Purpose of Use of Personal Information
We use the information received only as necessary to respond to and communicate about inquiries, consider research collaboration, and provide the site safely. If another purpose becomes necessary, we will explain it in advance and seek consent where appropriate.
3. External Services and Third-Party Disclosure
When you use Google Forms, the information you enter and submit, together with information associated with access, is processed through Google’s services. Email passes through the service providers used by the sender and recipient. The hosting provider may process communications data necessary to deliver the site. Each provider handles information under its own terms and privacy policy. See the Google Privacy Policy for Google’s practices. Other disclosure is limited to cases involving consent, a legal requirement, or protection of life, physical safety, or property when obtaining consent is difficult.
4. Security Management of Personal Information
Specific security measures depend on the settings of the email, Google Forms, and hosting services used. Because no internet communication or external service can be guaranteed to be completely secure, do not send health information, identity documents, credentials, or confidential organizational information in an initial inquiry. If sensitive information must be handled, confirm an appropriate communication method before sending it.
6. Web Analytics Tools
The current distribution files do not contain Google Analytics or other web-analytics code. Whether the production environment uses access logs or analytics requires separate verification. If an analytics tool is introduced, this page will identify the tool, purpose, information collected, and opt-out method.
7. Changes to the Privacy Policy
Changes to this policy will be published on this page with the updated date.
8. Contact
For questions about this policy, use the email option on the Contact page. You may contact us by email if you prefer not to use Google Forms. Do not include sensitive information in the initial message.
